After you schedule and pay for your appointment, links to these online forms should automatically be emailed to you upon booking. However, if for any reason they were not emailed to you or you can no longer locate the email, you can use the links below to access the online forms.

ACCUSER’S CASE INFORMATION FORM

Links to these online forms will be emailed you and are mandatory for all lie detector tests.

  • No alcohol of any kind or amount for a minimum of 48 hours prior to your exam.

  • Get a good nights sleep. Be well rested.
  • Complete and submit all forms no less than 72 hours prior to your exam.

  • Bring or wear a short sleeve shirt.
  • Due to COVID-19 and social distancing, only the Subject being tested is permitted in the building.
  • Do not bring an digital devices such as smart watches, tablets or laptops.
  • Power off your cell phone prior to entering the building.
  • The final questions asked on the exam WILL be worded differently than what you put on your questionnaires to ensure the questions meet national guidelines.
  • A parent or legal guardian is required for those testing under the age of 18.
  • Taking a polygraph exam in real life is absolutely nothing like it is on TV.
  • Being nervous is very common and normal.
  • Do not arrive hungry. Eat prior to your exam.
  • Children of any age are not permitted unless being tested. Please make any applicable daycare arrangements.
  • No drugs of any kind or amount a minimum of 48 hours prior to your exam unless medically required and prescribed by your doctor.

  • Do take any medically required medication that was prescribed to you by your doctor.
  • Have a valid, government issued ID ready upon arrival.
  • Due to COVID-19 and social distancing, a face mask is required at all times.

  • Leave all weapons at home. No weapons of any kind are permitted and you may be subject to search.
  • If you wear glasses to read, ensure to bring your glasses with you.
  • Arrive 15 minutes to your appointment.
  • If you arrive more than 15 minutes late, your appointment may be rescheduled or cancelled without refund.
  • There will be 1- 4 relevant, direct "Yes or No" questions asked on the polygraph. 

  • There will be 1 relevant, direct "True or False" question asked in a variety of different ways on the EyeDetect.
  • A detailed reported will be emailed to you usually within 24 hours of the exam and will contain the questions, answers and results.
